I'd like to request disclosure for family court proceedings about myself
Thank you. Just follow these steps to apply.
Step 1: Download the London request form (LRF) for private individuals representing themselves.
Step 2: Print and complete the LRF.
You' ll need to include:
You will receive an invoice for payment once the request has been completed. Download a list of the fees and charges. The Standard Disclosure Fee will be reviewed annually.
Step 3: Post your completed LRF and ID documents to the address below at least 20 days before the scheduled hearing date:
