I'd like to request disclosure for family court proceedings about myself

Thank you. Just follow these steps to apply.

 

Step 1: Download the London request form (LRF) for private individuals representing themselves and our LRF guidance notes.

 

Step 2: Print and complete the LRF.

You' ll need to include:

  • original copies of two forms of ID, one of which must be your passport, driving licence or identity card.

Step 3: Post your completed LRF and ID documents to the address below at least 20 days before the scheduled hearing date:
